First Visit and what to expect

Your first visit will likely be a consultation appointment in Dr. Davenport's office. 

You will need to bring

• a valid Health Card 
• a current list of any medications you are taking
• You must confirm your appointment 1 week prior to the date of the scheduled appointment

Please be advised that as of July 1, 2020, red and white health cards will no longer be accepted as proof of Ontario Health Insurance Plan ( OHIP ) coverage for insured health services.

Telemedicine

Telemedicine appointments are required to bring the same documentation to the Telemedicine Host site as well as your appointment must be confirmed 1 week prior.

• In some circumstances we can offer telemedicine to you in your home, but this service is only available at the request of Dr. Davenport.
• If you are being seen via Telemedicine and have questions about your upcoming visit please call 807-285-4860 and choose option 2.

Appointments and Procedures

Unfortunately, changes sometimes occur with relatively little warning (even within hours of your scheduled appointment time). Should a schedule change affect this appointment, we will notify you of an alternate date & time. It is important to provide us with all of the telephone numbers at which you would want to be reached in the event of a last minute schedule change. If you change your address, telephone number or health card, please advise our office. We look forward to being involved in your care.

Please remember that preparation is different for each patient and you must consult with the specialist performing the procedure prior to starting any type of Preparation.
